What We Do
We are a community-based comprehensive psychiatric rehabilitation service provider for minors, adolescents, adults, and seniors with the sole purpose of lowering the impact and burden of mental diseases in society at large.
We train participants so as to acquire knowledge and skills that will help them become more independent, self-sufficient, and productive and be able to manage crises that may be faced with.
Our clients are assisted to access and benefit from mental health-related resources that they may require to achieve the desired goals and objectives.

Our mission is to provide individuals of different ages the support that they deserve in addressing their mental health issues. With our services, we aim to empower individuals to acquire the knowledge and skills that will enable them to live functional and self-sufficient lives.
Who Is Eligible?
- Must be referred by a licensed behavioral clinician
- Individual that receives DS medical assistance {Medicaid} and primary adult care (PAC)
- Must be receiving behavioral health treatment and experiencing problem either at home, school, at work, and in the community
- Be aged 6 and above
Requirements
- Personal identification
- Birth certificate (for children)
- Social security card
- Current medication and dosage
- Medicaid CAED
- Information for 3 emergency contacts
